Hi could you help me interpret my vitamin D levels?:

D-25 = 20 ng/mL

1,25-D = 49 pg/mL

I was diagnosed with Autoimmune Hepatitis in May 2003

*My current lab test also showed an elevated creatine kinase level = 243 U/L

-Was the 1,25-D sample was frozen for shipment? Yes

-How long have you been avoiding foods with vitamin D in them? Have not yet

-How long have you been avoiding sun/lights? Have not yet

-List all meds, supplements (both prescription and OTC) you have been taking and why. Taking azathioprine (Imuran) and Zoloft

-When and for how long have you taken prednisone or any other immunosuppressant in any form? Took prednisone from May 2003 - Jan 2004. Presently taking Imuran (also started it in May 2003)

-When and for how long did you take Vitamin D supplements (including any vitamin-mineral supplements), omega-3 supplements and/or any kind of fish oil?
Don't/haven't taken vitamin D supplements

-Were you taking Benicar when the sample was drawn? No

-Were you taking an
ARB or or an ACE inhibitor when the sample was drawn?
    No

Feedback on vitamin D results

25-D:  20 ng/ml
1,25-D:  49 pg/ml
Steroids/TNF drugs:  Yes
Olmesartan:   No

An interpretation of 1,25-D are only marginally useful after a patient has taken corticosteroids, anti-fungals, or TNF-alpha drugs in the last 60 days. For patients whose 1,25-D value is 49 pg/ml, we would typically expect the unsuppressed 1,25-D to be 10% to 30% higher than that, which certainly would indicate the presence of Th1 inflammation.
However, despite the suppression of 1,25-D by corticosteroids, anti-fungals, or TNF-alpha drugs, this level is still elevated, indicating the VDR dysfunction typically seen in Th1 disease.
The level of 25-D is just above the therapeutic range. This is a good place from which to initiate therapy concomitant with continued avoidance of ingested Vitamin D.
