Barney, I just ran the numbers on my vitamin D - obviously I am concerned with the comment at the end of the page. See below:

Steroids/TNF drugs:  No

Olmesartan:   No
The 25-D has built up to a level which is highly immunosuppressive. It will need to be weaned lower before test results can be meaningful, and before contemplating any therapy.

Despite the very high value of 25-D, the 1,25-D is high enough to show dysregulation of the VDR control metabolism. This is typically seen in people who are very sick, often with cardiac or pulmonary inflammation.

I am a newbie at this and with all the info, just trying to get a handle on some of the info.  There is so muchto read, learn and digest!  I am conccerned now that I need to make an appt with my asthma specialist.....  I just started the Benicar (20 mg) last week and have ordered the Mino - it will be here in 10 days, which at the time I am to increase the Benicar to 40mg. Some of the symptons I am having are: flu-like achiness, headaches, extreme fatigue, dizziness/lightheaded - I have ordered the NoIR glasses -do you feel any of these symptons could be from sun exposure? I am trying to stay out of the sun AMAP, but it is challenging. Also have started the lowcarb, noVitD diet.

(I am guessing these are herx)

Benicar will also help to low D, but you need your NOirs immediately because as you take benicar, light sensitivity should start and could give you a hard time.

D levels will drop eventually, I wouldnt be very alarmed, because you are in the correct path

Marion, at night I am having headaches and some shoulder/back pain; also, all the time I can only do one thing for a bit, then rest.  I feel achy and have no energy.  This sounds like what I am reading on other posts, but when do you start gaining strenght?  Are you on the program due to your RA?  Do you see some improvement yet?  The websites are awesome and a ton of help!  I feel so blessed to be able to have this kind of support.  Thanks a million. It sounds like you are ramping the Benicar.  In the phase  one guidlines you will see that you should start Benicar at full dose of 40 mg every 6 hours.

What does "ramping the Benicar" mean and I will go ahead and get on the Benicar full strength - it sounds like I need to.....  I have read the Phase One GL but need to read again, and again....  I have not been able to read much at one time as my eyes get extremely tired.   Will get my hardcopy now and get busy since I had a good nap.  Thanks Gene!

Hi Joni,

Ramping means to start out with low dose and slowly increase to desired dose.  However, that is not recommended for Benicar.  Benicar must be taken at the desired dose of 40 mg every 6 hours or more often if the IP becomes intolerable.  If that happens take an extra 40 mg Benicar tablet with an additional half tablet dissolved under the tongue.  You need to be sure you have extra Benicar to be able to do this as it is very important. Benicar is very important to control the IP.

High values of 1,25-D (such as you have) indicate the infection is involved with organs profused with blood such as the heart.   You need to be aware of this and be sure to stay on the Benicar until your symptoms allow you to add small doses of the antibiotic (abx).  It that case we do ramp the abx starting with 25 mg of minocycline and increase to 100 mg in 25 mg increments based on the IP tolerance.

You must have patience as the MP can not be rushed.  Maintain tolerable IP by slowly ramping the abx.  If the IP becomes intolerable reduce or stop the abx and increase the benicar. 

Keep asking questions until you are comfortable with the MP.

You should check your calcium levels in the urine and blood, including ionized calcium and albumin, as you are in risk of having hypercalcimia.

You should also test PTH and PTHrp to check for parathyroid status.

You should avoid all sources of vitamin D, obvioulsy.

Just my advice as I also am a "high" 1,25D person.... 235pmol (around 96 pg) and I have occasional hypercal.

Needles to say: If your PTH is suppressed you should start the MP as soon as possible. If PTH is high, you should investigate hyperparathyordism.

(I am not a doctor and my advice are ment as input for you to bring to your doctor who obvioulsy should bee aware of these tests and why I propose them)

I'm new.  Does Omega 3 fish oil supplement have Vit D?  I've been taking it for several years for low HDL cholesterol which it corrects.

If you go back to the previous page "Help with Understanding D-metabolites tests" and scroll to the top of the page, click on "New topic" and post your D-metabolites there.
Also, you can go to "Ask Your Questions in Here" and click on "New Topic" and post a new question. That way, it won't get lost at the end of a different thread where few people will see it unless they happen to be watching that thread.

And, yes, fish oils are high in Vitamin D. If you've been taking them for a long time it would affect your D levels. That's why they ask about supplements when you post your D-metabolites results.

Are you new to the MP? And, have you had both 25-D and 1,25-D levels tested?
